The new survey is only required if you are an employee or a student reporting:

• A positive COVID-19 test
• COVID-like symptoms (symptoms from a known or chronic condition (e.g. allergies, asthma, etc.) are not considered COVID-19 symptoms for this report)
• Exposure to someone who tested positive for COVID-19 (close contact with a known positive person for more than 15 minutes in a 24 hour period)
• Completion of your required COVID-19 test (You are required to submit COVID test information if you have an approved exemption or deferral, if you are not currently up to date with your COVID vaccinations OR you are requesting an early return after an exposure or COVID infection)
